is hiring for a Community Resources Specialist! The Community Resources Specialist will work directly with customers to meet their needs and help to get them back into the community.
This will include:
Home visits Shopping Providing assistance in identifying and securing a safe home in the community Providing support in obtaining furnishings, household items, DME and items necessary for transition to live independently Assistance with finding community resources and supports for HCBS services Create a budget with customer of deposits, items needed, monthly rent for a successful move back into the community Providing follow up after move is complete:
three days after move and 30 days after move Coordinate move date:
landlord, utilities, HCBS services, deposits, household items, medications, follow up doctors appointment Provide training and materials for customers to understand their rights as the leave the nursing home and back into the community Working with individuals with Autism to provide Autism Services Incoming calls for TCS, I&A and FMS Senior Care Act Signs Ups, and bi annual check ins in person, monthly follow ups Authorization and Service Plan Tracking/interactions with MCO's New FMS Sign ups (TA, BI, PD, FE, IDD) Follow up with new sign ups Skills Training Review/ Assist with new hire packets, Over 40, Exhausted ISP, Clocking in and out app and calling in option , time sheets, ECS, over 12 hours, monitor live in exceptions, interviewing hiring and firing scheduling, authenticare app, activity codes, etc) Identifying those that are struggling and providing extra tools/training Reviewing PA packets as needed BI, TA Payroll Monitor Live In Exceptions ECS Others duties as assigned Quick Apply.
Estimated Salary: $20 to $28 per hour based on qualifications.
